The Low BP That Needed No More Fluid
The presentation
A breathless patient with hypotension received repeated fluid boluses and became more hypoxic.
The first explanation was plausible, but the pattern did not fully fit. A deliberate second look focused on physiology, medication/exposure history, serial observations and the clue that the initial diagnosis could not explain.
The turn
Raised JVP, pulmonary congestion and poor LV function showed that preload was already excessive.
Diagnosis
Cardiogenic shock
Why this case matters
Shock is not synonymous with fluid deficiency.
Take-home message: When the clinical course contradicts the working diagnosis, stop escalating the same treatment and rebuild the problem from first principles.
